Structural Elements, an orthopedic wellness franchise based in Hagerstown, Maryland, said on October 15, 2026 that it is recruiting experienced multi-unit investors as it pursues a target of 125 clinics over the next five years, according to a company announcement distributed by PR Newswire.
The company sets its initial franchise fee at $65,000, dropping to $52,000 for each extra clinic opened under a multi-unit deal. It estimates the full startup outlay at $703,000 to $1.4 million for suburban sites of 5,000 to 7,000 square feet. The brand is registered to sell franchises in Maryland, Wisconsin and Florida, and is targeting nearer-term openings in Maryland, Pennsylvania and North Carolina.
Structural Elements has sold franchises since 2016 and opened its first location in Hagerstown in 2013. Its current network covers clinics in Maryland and Wisconsin plus 11 practitioners who own practices inside those clinics under three-year agreements, a structure the company began using in 2019. Clinic owners handle scheduling, billing, marketing and the front desk, and earn revenue from space fees, recovery memberships, retail sales, workshops and corporate wellness programs. Clinical credentials are not required of clinic owners.
Founder and chief executive Douglas Bertram said the company's model has been developed over more than 10 years with patients and providers.
